Culture Service Program Participants,
Rather than waiting for notification by traditional post, completion of this form will
allow you to receive your sterilizer test results by email the day incubation is completed.
Email notification has the added benefit of never getting lost in transit.
Propper will not share your address with third parties or fill your inbox with spam.
Please complete the E-mail Notification Signup Form as accurately as possible.
Spore test results will be sent to the email addresses you provide below.
As such, please be sure that the email(s) you provide below will be checked regularly.
We identify customers based on the telephone number and zip code written on their Sterilization Report Forms
(i.e. your mail-in test form). We strongly recommend that on every mail-in test form, you provide us
with the same phone number for each facility. However, if you need to have multiple phone numbers associated
with our Culture Service Program, we ask that you fill out additional E-mail Notification Signup Forms to
register those phone numbers.
If you ever wish to update your contact information, you may simply fill out another E-mail Notification Signup Form
to reflect the changes. We will send you a confirmation letter as soon as we process your new information.
